If you were a full-time employee or eligible for coverage under Scott Credit Union’s employer-sponsored health plan during the prior calendar year, you can request a copy of your Form 1095-C at any time before October 15, 2026.
You may request your Form 1095-C using one of the following methods:

Upon request, Scott Credit Union will provide your Form 1095-C by the later of:
- January 31 of the year following the calendar year the form relates to, or

This notice will remain available on this website through October 15, 2026 to allow you time to request your Form 1095-C.

Scott Credit Union created a community Volunteer Incentive Program (VIP) to show how important community involvement is to us and to encourage our staff to volunteer their time supporting local efforts. We offer our staff a chance to win prizes for volunteering. We’re proud of our staff for all that they do to give back to the community, and we show our support and gratitude for their service with tangible rewards.
Scott Credit Union has made giving back to its communities a major priority. In the past fifteen years, we donated nearly $3.9 million and employees volunteered nearly 25,000 hours to a variety of local community events and organizations throughout the region.
